| Abstract | Istražili smo genotoksični i citotoksični učinak inhalacijskih anestetika sevoflurana, halotana i izoflurana, citostatika cisplatine te združeni učinak inhalacijskih anestetika i cisplatine na zdrave stanice Swiss albino miševa te zdrave i tumorske stanice miševa nositelja Ehrlich ascitesnog tumora (EAT) na temelju kvantitativnih pokazatelja molekularnih i staničnih oštećenja kao i kliničkih biokemijskih i hematoloških pokazatelja. Cilj istraživanja bio je ustanoviti da li združeno djelovanje inhalacijskih anestetika i cisplatine pojačava ili slabi njihov pojedinačni učinak na limfocite periferne krvi, stanice jetre, bubrega, mozga te stanice Ehrlich ascitesnog tumora i koji od navedenih inhalacijskih anestetika kod višekratnih anestezija ima najmanju toksičnost na zdrave stanice te povećanu toksičnost na stanice tumora.
Miševe smo podijelili u dvije skupine; zdrave miševe te miševe nositelje tumora (injicirane intraperitonealno (i.p.) s 2 X 106 stanica EAT). Stanice EAT održavali smo u Swiss albino miševima serijskim presađivanjem stanica i.p. u trbušnu šupljinu miša u periodu od 7-9 dana u obliku ascitesa. Obje skupine miševa podijelili smo u osam podskupina ovisno o tome da li smo ih opetovano anestezirali sevofluranom, halotanom ili izofluranom, te da li smo im injicirali intraperitonealno cisplatinu.
Upite na postavljene ciljeve razjasnili smo rabeći slijedeće metode: komet test, mikronukleus test, analizu postotka tumorskih stanica u apoptozi i nekrozi, određivanje broja živih i mrtvih tumorskih stanica u peritonealnoj tekućini, te određivanje hematoloških i biokemijskih odrednica krvi pokusnih životinja.
Rezultati komet testa pokazuju da sva tri inhalacijska anestetika izazivaju oštećenja DNK limfocita periferne krvi, stanica mozga, jetre, bubrega te Ehrlich ascitesnih tumorskih stanica. Najjača genotoksična oštećenja prouzročena inhalacijskim anesteticima vidljiva su na stanicama mozga, posebice nakon ponavljanih anestezija halotanom. Sevofluran ima najjači genotoksični učinak na limfocite periferne krvi, a halotan na stanice jetre. Sevofluran i izofluran znatno jače oštećuju DNK stanica bubrega od halotana. Limfociti periferne krvi, stanice mozga, jetre i bubrega miševa s injiciranim tumorskim stanicama imaju znatno veća bazalna oštećenja DNK nego istovrsne stanice životinja bez tumora. DNK stanica Ehrlich ascitesnog tumora pokazuje najveću osjetljivost na ponavljane anestezije halotanom.
Kombinacija cisplatine i inhalacijskih anestetika ima znatno jači genotoksični učinak na limfocite periferne krvi, stanice jetre i bubrega u odnosu na učinak samih inhalacijskih anestetika, međutim kombinacija cisplatine i inhalacijskih anestetika ima manji genotoksični učinak na EAT stanice u odnosu na pojedinačni učinak sevoflurana, halotana (p<0,01) i izoflurana (p<0,05). Nadalje, anestezije halotanom prouzročile su najveći broj mikronukleusa u retikulocitima periferne krvi miševa. Višekratna anestezija halotanom i sevofluranom smanjuju broj živih EAT stanica u ascitesu miševa u odnosu na kontrolu, dok anestezija izofluranom pospješuje rast stanica tumora. Smrt stanica prouzročena cisplatinom temelji se na procesu apoptoze u oko 40% stanica tumora, dok anestezija halotanom i sevofluranom potiče apoptozu u 10% stanica tumora. Združeni učinak cisplatine i inhalacijskih anestetika povećava broj ukupno mrtvih stanica u ascitesu miševa. Halotan i izofluran smanjuju broj leukocita u perifernoj krvi, a kombinacija halotana i cisplatine dovodi do najjače leukopenije i neutropenije te djeluje hepatotoksično. Sukladno rezultatima našeg istraživanja halotan ima najjači genotoksični i citotoksični učinak na tumorske stanice, međutim obzirom na njegovu toksičnost na zdrave stanice prikladniji anestetik za anesteziju miševa s tumorom je sevofluran, dok ponavljane anestezije izofluranom povećavaju rast tumorskih stanica.
Dobiveni rezultati upućuju na potrebu daljnjih istraživanja vezano uz genotoksičnost i citotoksičnost inhalacijskih anestetika, kao i njihove međureakcije s nekim drugim citostaticima koji su u svakodnevnoj upotrebi kod bolesnika s malignim bolestima. Također je potrebno bolje proučiti mehanizam na koji dolazi do pojačavanja ili slabljenja učinka citostatika na tumorske stanice zbog međureakcije s anesteticima.
Buduća istraživanja bilo bi korisno usmjeriti na analizu popravaka DNK zdravih stanica nakon ponavljanih anestezija inhalacijskim anesteticima i združenog djelovanja s citostaticima metodama komet testa i nekim drugim citogenetičkim testovima (strukturne aberacije kromosoma, izmjene sestrinskih kromatida, mikronukleus test i dr.) kao i pratiti mogućnost nastanka posljedica vidljivih tek kroz duži vremenski period.

| Abstract (english) | In this study the genotoxic and cytotoxic effects of inhalation anaesthetic isoflurane, sevoflurane and halothane, with/or without cytostatic cisplatine were investigated on healthy Swiss albino mice cells and tumour cells of Ehrlich ascites tumour (EAT) carrier mice using quantitative analysis of molecular and cell structure damage and clinical biochemical and haematological assays. The goal of this study was to explore whether inhalation anaesthetics together with cisplatine enhance or decrease their effects on peripheral blood lymphocytes, hepatocytes, renal cells, neurons and Ehrlich ascites tumour cells. Additional goal was to determine which of those anaesthetics induced the least toxicity on healthy cells and which one increased toxic effect of cisplatine on tumour cells.
The mice were divided in two groups: healthy control mice and mice carriers of tumours (i.p. injection with 2 X 106 EAT cells). EAT cell line were kept alive in albino mice by serial passages intraperitoneally every 7-9 days in ascites fluid. Those two groups were subdivided into 8 subgroups each, based on repeated anaesthesia with sevoflurane, halothane or isoflurane, or i.p. injection of cisplatine. Cells were collected and analyzed by comet test, micronucleus test, determination of percent of tumour cells in apoptosis and necrosis, by counting alive and dead tumour cells in peritoneal fluid and by haematological and biochemical tests from blood of experimental animals.
The results of comet test showed that all three inhalation anaesthetics damaged DNA of lymphocytes, neurons, hepatocyte, kidney and EAT cells. The strongest genotoxic damage is seen in neurons after repeated treatment with halothane. Sevoflurane exhibited the strongest genotoxic effect on lymphocytes and halothane had the most deleterious effect on hepatocytes. Sevoflurane and isoflurane damage DNA of kidney cells more that halothane. Lymphocytes, neurons, hepatocytes and kidney cells from mice with tumour had much higher basal DNA damage that same type cells from control animals without tumour. Combination of cisplatine and inhalation anaesthetics had much stronger genotoxic effects on lymphocytes, renal and liver cells than inhalation anaesthetics per se. However, combination of cisplatine and inhalation anaesthetics has less genotoxic effect on EAT cells than individual effect of sevoflurane, halothane (p<0.01) and isoflurane (p<0.05). Furthermore, anaesthesia with halotane induced the highest number of micronuclei in reticulocytes of peripheral blood. Repeated anaesthesia with halothane and sevoflurane decreased number of living EAT cells in ascites compared to control, while anaesthesia with isoflurane increased the tumour cells growth. Cisplatine induced apoptosis in 40% of tumour cells, while anaesthesia with halothane and sevoflurane induce apoptosis in 10% of tumour cells. Cisplatine and inhalation anaesthetics together increased the number of dead cells in ascites of mice. Halothane and isoflurane decreased the number of leucocytes in blood and combination of halothane and cisplatine induced leucopoenia and neutropaenia and had hepatotoxic effects. The strongest genotoxic and cytotoxic effects on tumour cells has halothane, however due to its toxicity on healthy cells more appropriate anaesthetic for mice with tumour is sevoflurane, while repeated anaesthesia with isoflurane increased the growth of tumour cells.
The results of this study emphasize the needs for further studies exploring genotoxicity and cytotoxicity of inhalation anaesthetics, and their interactions with other cytostatics which are commonly used in routine clinical practice in patients with malignant disease. It is necessary to investigate in greater details the mechanisms of increased or decreased effects of cytostatics on tumour cells due to interaction with inhalation anaesthetics.
