Abstract
Spinal deformities are a growing problem in today’s society which has a prevailing sedentary lifestyle. Early detection of spinal deformities is a very important factor in progression prevention and deformity correction. The aim of the doctoral research was to develop methods that can overcome simple visual examinations and diagnostic techniques, and to reduce the susceptibility to human error factor and the physician’s expertise level. In this dissertation, a 3D scanning system for a dynamic reconstruction of the back surface is presented. The system can successfully reconstruct the back surface when the patient is in the upright standing position and while performing forward bending. The proposed method for automated detection of the spinal curve on the back surface, based on the surface curvature analysis and the graph theory algorithm, can be used for all positions of the back obtained during forward bending. The 3D scanning system and the method for spinal curve detection are used to define a new ASM parameter for the automated deformity assessment which proved to be a very good alternative to the standard scoliometer. The dynamic analysis of the spinal curve enables the diagnostics of the spinal mobility by using proposed 2D visualizations. The original scientific contribution of the thesis are: 1. method for detecting the asymmetry curve of the back under dynamic conditions in scoliosis diagnostics, 2. parameters for analysis of the spinal deformity degree in scoliosis diagnostics, and 3. method for non-invasive analysis of spinal mobility.
Abstract (croatian)
Uslijed pretežito sjedilačkog načina života, iskrivljenje kralježnice rastući je problem današnjeg društva. Pri prevenciji progresije i započinjanja uspješnog liječenja, veoma bitan faktor jest rana detekcija iskrivljenja kralježnice. Cilj ovog doktorskog istraživanja bio je razviti metode koje mogu nadići jednostavne vizualne preglede i dijagnostičke postupke, ali i umanjiti podložnost faktorima ljudske pogreške i razine iskustva liječnika. U doktorskom radu predložen je sustav za dinamičku 3D rekonstrukciju površine leđa, kojim je moguće rekonstruirati površinu leđa pacijenata u uspravnom položaju, ali i u svim položajima tijekom izvođenja pretklona unaprijed. Predložena metoda za automatsku detekciju krivulje kralježnice na površini leđa, zasnovana na analizi zakrivljenosti površine i na korištenju algoritma iz područja teorije grafova, primjenjiva je na svim položajima leđa rekonstruiranih pri pretklonu. 3D sustav i metoda za detekciju krivulje kralježnice iskorišteni su za definiranje parametra ASM za automatsko mjerenje stupnja iskrivljenja kralježnice, koji se pokazao kao prikladna alternativa dijagnostici standardnim skoliometrom. Dinamičkom analizom krivulje kralježnice omogućena je i dijagnostika pokretljivosti kralježnice korištenjem predloženih 2D vizualizacija. Izvorni znanstveni doprinos disertacije obuhvaća: 1. metodu detekcije krivulje asimetrije leđa pri dinamičkim uvjetima u dijagnostici skolioze, 2. parametre za analizu stupnja deformacije kralježnice u dijagnostici skolioze, te 3. metodu neinvazivne analize pokretljivosti kralježnice.
