Neural response telemetry (NRT) has been available for over 20 years. Impedances, evoked compound action potential (ECAP) threshold, amplitude growth factor (AGF) and some seldom used parameters are obtainable. Programming of the speech processor is done individually. Hearing threshold (T levels) and comfort thresholds (C levels) are basic parameters that must be set for each user. The interval between the two is the dynamic range (DR). The aim of many researchers is to find the algorithm that would automatically program the speech processor and set T and C levels based on NRT data. The objective of this study is to determine whether intraoperative NRT on the apical portion of the cochlea correlates better to final T and C levels comparing to NRT performed on the basal and middle portion. The aim is to show if intraoperative NRT on any of the three portions of the cochlea, compared to each other, has a better predictive value for final T and C levels. Correlation of AGF to T and C levels, AGF to DR and the dynamic of change of objective parameters from intraoperative values to those measured 12 months after the surgery is also shown. Intraoperative NRT data of 31 children were compared to NRT data and subjective T and C levels collected 12 months following surgery. All candidates were younger than 18 years and all were implanted with Nucleus Cochlear CI 24R or RE cochlear implant. Complete insertion of the electrode and presence of all required NRT data on electrode 19, 11 and 5 intraoperatively and 12 months later in addition to T and C levels, were set as inclusion criteria. Electrode 19 is representing the apical, 11 the middle and 5 the basal part of the cochlea. No statistically significant difference in prognostic value of NRT data for final T and C levels collected at the apical portion of the cochlea compared to the other two was found. No statistical difference was found comparing any of the three portions of the cochlea. Evaluation of changes of intraoperative NRT data showed that there is a statistically significant decrease of all parameters 12 months following surgery. The biggest decrease occurred on electrode 19. There is no statistically significant difference between the electrodes for ECAP and AGF. Impedances decreased statistically significant less on electrode 5 compared to the other two. Statistically significant correlation of AGF to T level (r=0,45, p=0,011) and AGF to DR (r=-0,458, p=0,010) was found. Again, difference between the electrodes was not significant. In the same manner intraoperative ECAP threshold shows statistically significant correlation to C levels on electrode 19, to T and C levels, and DR on electrode 11, and T and C levels on electrode 5. Hypothesis was not confirmed. NRT on apical segment of the cochleae does not correlate significantly better to final T levels, C levels and DR compared to NRT on middle and basal electrodes. Statistically significant correlations that were found must be taken with precaution. Correlation is weak to moderate, with large standard error and low precision of prediction.
