Abstract
UVOD: Križobolja i radikulopatija predstavljaju čest javno zdravstveni problem. Bolest koja je najčešće uzrokovana hernijom intervertebralnog diska uzrokuje kompresiju neuralnih struktura, poglavito spinalnih korijena. Ozbiljni neurološki deficiti, kod kojih ne dolazi do oporavka po konzervativnom tretmanu, dovode do postavljanja indikacije za operacijsko liječenje. Ponovni razvoj diskalne hernije na operiranom nivou (rehernijacija) najčešća je neželjena posljedica operativnog zahvata (javlja se kod 3-15% bolesnika). Također se kao posljedica operacijskog zahvata mogu razviti kronični bolni sindromi koji su posljedica progresije degenerativnih promjena i promjena u biomehaničkim odnosima operiranog segmenta. HIPOTEZA: Ugradnja proteze za pojačanje i rekonstrukciju anulusa fibrosusa (anuloplastika) herniranog diska omogućava kirurgu poštedniji operacijski zahvat uz diskektomiju kojom se odstranjuje manje diskalnog tkiva. Rekonstrukcija anulusa dovodi do smanjene incidencije rehernijacije. Očuvanje visine intervertebralnog diska dovodi do smanjivanja utjecaja operacijskog zahvata na biomehaniku operiranog segmenta te posljedično do boljeg kliničkog ishoda bolesnika..
Abstract (english)
Back pain and radiculopathy represent a frequent public health issue. The condition that is most often caused by intervertebral disc herniation leads to the compression of neural structures, mainly of spinal roots. Serious neurological deficits, that do not improve following conservative treatment, are an indication for surgical treatment. A recurrence of disc hernia at the surgically treated level is the most frequent adverse consequence of surgical treatment (3-15% of patients). Chronic pain syndromes are another adverse consequence of surgical treatment; this stems from degenerative changes and changes in biomechanical relations of the surgically treated level. HYPOTHESIS: Implanting a prosthesis for reinforcement and reconstruction of the annulus fibrosus (anuloplasty) of the herniated disc, enables the surgeon to perform a less invasive surgical procedure, which involves removal of less disc tissue. The reconstruction of the annulus brings about a reduced incidence of reherniation. Preserving the disc height of the intervertebral disc reduces the impact of the surgical procedure on the biomechanics of the surgically treated segment, thus also leading to a better clinical outcome.
