Clinical medicine Abstract Zbog rasta incidencije malih KCB, kod pacijenata sa komorbiditetima došlo je do primjene perkutanih ablativnih metoda. MVA bubrega još uvijek nije uvrštena u terapijske smjernice te se smatra eksperimentalnom. Iako postoje studije koje dokazuju učinkovitost i sigurnost MVA-e, nedostaje studija koje bi direktno uspoređivale PN i perkutanu MVA.
Ciljevi istraživanja bili su usporediti stopu lokalnog recidiva, ukupno preživljenje, preživljenje bez metastaze i preživljenje specifično za karcinom nakon perkutane visokoenergetske mikrovalne ablacije pod kontrolom kompjuterizirane tomografije i parcijalne nefrektomije u terapiji karcinoma bubrega stadija T1a te usporediti učestalost komplikacija kao i utjecaj MVA i PN na bubrežnu funkciju. U retrospektivnu studiju bilo je uključeno osamdeset pacijenata, od 1. siječnja 2015. do 30. lipnja 2018. s KCB stadija T1a. Od pacijenata indiciranih za PN, izabralo se pacijente koji prema veličini tumora i kompleksnosti tumora odgovaraju skupini liječenih s MVA. Uključeni pacijenti, su radiološki i klinički praćeni najmanje 12 mjeseci nakon zahvata. Zahvat MVA izvodio se kod svih pacijenata perkutanim pristupom, pod kontrolom CT-a.
Nije se dokazalo postojanje statistički značajne razlike između ove dvije terapijske metode u ukupnom preživljenju, preživljenju bez lokalnog recidiva, u preživljenju specifičnom za karcinom niti u preživljenju bez metastaza i bez bolesti. Prosječni postotak gubitka bubrežne funkcije, iznosio je –8,9 ± 6,0 % za MVA skupinu i –21,7 ± 8,2 % za PN skupinu, što predstavlja statistički značajnu razliku (P < 0,001). Zabilježena je statistički značajna razlika u učestalosti komplikacija. Nije dokazana hipoteza da MVA ima bolje onkološke ishode od PN-a u terapiji KCB-a stadija T1a, ali dobiveni rezultati ukazuju na jednaku vrijednost ovih dviju metoda. Dokazana je prednost MVA-e u smislu manje učestalosti komplikacija i manjeg utjecaja na bubrežnu funkciju. Može se zaključiti da perkutana MVA može predstavljati jednakovrijednu alternativu zlatnom standardu kirurške PN kod pacijenata sa znatnim komorbiditetima, ali i kod ostalih pacijenata s malim tumorima bubrega, zbog dokazanih prednosti očuvanja funkcije bubrega.
Abstract (english) An increase in the incidence of small RCC lead to the development of percutanoues ablative therapies in patients with comorbidities. MVA has not been introduced into therapy guidelines and is still considered to be experimental. Despite existing literature confirming the efficacy and safety of MWA, there is still a lack of studies that directly compare PN and percutaneous MWA.
The aims of this study were: To compare local recurrence rates, overall survival, metastasis-free survival and cancer specific survival after percutaneous high-energy CT-guided MWA and PN in the therapy of T1a stage RCC and to compare complication rates and the effect on renal function.
The retrospective study involved 80 patients with T1a stage RCC from January 1st 2015 to June 30th 2018. Patients who were treated surgically were chosen, according to their tumour size and complexity, to match the patients treated with MWA. All patients were under radiological and clinical follow-up for a period of at least 12 months. The results did not show statistically significant difference between MWA and PN in overall survival, recurrence-free survival, disease-free survival, metastasis-free survival or disease-free survival. Decrease in the glomerular filtration rate was significantly lower after MWA and a significantly higher complication rate found after PN compared to MWA.
The hypothesis that MWA has better oncological outcomes compared to PN for T1a RCC has not been proven in our study, however the results point to equal value of both therapy approaches. It can be concluded that MWA can be used as an equally successful therapeutic tool in small RCC, when compared to the golden standard of partial nephrectomy, in patients with severe comorbidities, but also in other patients due to its nephron sparing qualities.
