Abstract
KOPB jedan je od vodećih uzroka smrti u razvijenim zemljama svijeta, a glavni uzroci mortaliteta i morbiditeta u KOPB bolesnika su akutne egzacerbacije i kardiovaskularne bolesti. S obzirom na učestalost egzacerbacija definiran je fenotip „učestalih egzacerbatora“ , kojeg uz teži klinički tijek, prema nekim pokazateljima karakterizira i povećan rizik za kardiovaskularni mortalitet. Mišljenja smo da sistemska upala ima ključnu ulogu u patogenezi KOPB-a, a moguća veza između povećanog kardiovaskularnog mortaliteta i sistemske upale kod KOPB bolesnika je endotelna disfunkcija. U ovom istraživanju po prvi puta provedeno je sustavno istraživanje endotelne funkcije u dvije fenotipski različite skupine KOPB bolesnika uz istovremenu procjenu endotelne funkcije FMD indeksom i plazmatskim biljezima, procjenu sistemske upale, plućne funkcije i funkcionalnog statusa bolesnika. U istraživanje smo uključili 117 sudionika; 82 bolesnika sa dijagnozom KOPB-a koje smo prema kriteriju učestalosti egzacerbacija podijelili u dvije skupine; u skupinu „učestalih egzacerbatora“ i skupinu bolesnika sa KOPB-om koji nemaju česte egzacerbacije. Kontrolnu skupinu činilo je 35 zdravih sudionika, aktivnih pušača. U svih ispitanika provedeno je mjerenje tjelesne sposobnosti 6-MTH, ispitivanje plućne funkcije, procjena simptoma putem CAT upitnika, mMRC skale zaduhe i određen je BODE indeks. U uzorcima krvi određeni su markeri sistemske upale ( L, CRP i fibrinogen), markeri endotelne funkcije (vWF i ET-1), te je kod svih bolesnika učinjen FMD test. Dobiveni rezultati su statistički obrađeni. Utvrđeno je da između dvije fenotipski različite skupine bolesnika sa KOPB-om postoji statistički značajna razlika u stupnju endotelne disfunkcije i sistemske upale, prisutnosti simptoma bolesti kao i u plućnim funkcijama, a nije nađena statistički značajna razlika u tjelesnoj sposobnosti. Fenotip „učestalih egzacerbatora“ karakterizira značajno narušena plućna funkcija, uz viši stupanj sistemske upale i endotelne disfunkcije. Vjerujemo da će naše istraživanje predstavljati značajan doprinos u boljem razumijevanju, ranom otkrivanju i adekvatnijem liječenju ove fenotipske skupine bolesnika sa KOPB-om.
Abstract (english)
The „frequent egzacerbator“ is a COPD phenotype characterised with severe clinical course and with increased risk for cardiovascular mortality. The systemic inflammation play a key role in the pathogenesis of COPD and endothelial dysfunction is a suspected link between increased cardiovascular mortality and systemic inflammation in COPD patients.So far, systematic studies of endothelial dysfunction in two phenotypically distinct groups of COPD patients has not been done. The study recruited 117 participants. COPD patients were divided into two groups according to the criteria of frequency of acute egzacerbation: 41 frequent egzacerbator and 41 infrequent egzacerbator. The control group consists of 35 healthy smokers. In all three groups were determined markers of endothelial dysfunction ( ET-1,vWF) and of systemic inflammation ( WBC, fibrinogen, CRP). We conducted pulmonary function test and 6-MWT-minute walking test, and calculated BODE indeks. Endothelial function was assessed by FMD. The results were statistically analyzed. Frequent exacerbator had more severe airway obstruction and higher degree of sistemic inflammation and endothelial dysfunction, with higher symptom score and BODE index.We believe that our research will represent a significant contribution to better understanding, early detection and adequate treatment of these groups of COPD patients
